|
Works independently under general supervision of Director of Health & Wellness. Responsible for coordinating the Office of Minority Health PAC grant, implementing community & clinical services for diabetes and cardiovascular disease management and prevention. The primary purpose is to plan and implement community and clinical health promotion/disease prevention programs to facilitate voluntary interventions that reduce the risk of diabetes and cardiovascular disease. Coordinate with team to provide optimal care for patients. Supports the OMH PAC grant: Healthy Tulsa Pathways (HTP) program. |

1. |
Coordinate program planning & implementation that supports the goals and objectives for the Healthy Tulsa Pathways (HTP) project. |
|

2. |
Facilitate regular management team meetings of IHCRC and Contractual Partners staff; facilitate regular quarterly meetings of the Healthy Tulsa Pathways workgroup of the Family Health Coalition. |
|
|
|
|
|
3. |
Serve as community partner liaison; meet with partners and coalition members on a regular basis. Maintain effective working relationship with partnership agencies. |
|
|
|
|
|
4. |
Develop community relationships that result in our ability to expand program to additional health and nutritional educational and physical activity sites. |
|
|
|
|
|
5. |
Conduct culturally appropriate knowledge dissemination of health promotion/disease prevention concept and techniques; conduct culturally appropriate social marketing that results in reduction in diabetes, cardiovascular disease and commercial tobacco use. |
|
|
|
|
|
6. |
Help schools/communities create standards that reflect a positive approach to healthy lifestyles, such as improved access to healthy food choices and walking trails. |
|
|
|
|
|
7. |
Assist as needed with community projects such as community gardens, fitness programs, nutrition programs, environmental and policy changes that will improve the health and well-being of the community. |
|
|
|
|
|
8. |
Develop effective approaches to recruit potential patients, including minority youth and families, into the program and maintain their continued involvement. |
|
|
|
|
|
9. |
Conduct ongoing project evaluation. Assess client satisfaction levels and beliefs regarding effectiveness and cultural appropriateness of selected interventions. Participate in evaluation process with Office of Minority Health & Tulsa Health Department as needed to meet project goals. |
|
|

|
|
10. |
Maintain awareness of current research in the field of prevention of diabetes and cardiovascular disease. |
|
|
|
|
|
11. |
Is familiar with and adheres to the Compliance plan and all related policies. Reports questionable activity to Compliance Officer. Maintains accountability for complying with all applicable standards, laws, regulations and procedures governing the delivery of healthcare services and associated processes. |
|

|
|
12. |
Assist with clinical case management as needed. |
|
|
|
|
|
13. |
Attend OMH grant meetings as required. |
|
|
|
|
|
14. |
Displays strong written and oral communication skills; responsible for grant quarterly reporting as determined in grant guidance. |

Knowledge, Skills, Abilities |
|
Master’s degree in public health, education or related field. Minimum of 3 - 5 years experience in health education and case management. Experience in minority public health care setting preferred. |
|
Experience in or a professional background relevant to community based health promotion programs and coalition management. |
|
CPR certification. |
|
Ability to demonstrate a special understanding of Indian culture. Familiarity with Native American health care trends. |
|
Intermediate computer skills. Proficient in MS Office, Excel, Power Point and/or additional applicable software programs. |
|
Strong written and oral communication skills. Excellent interpersonal skills. |
|
Ability to multi-task and work independently, demonstrating initiative and accuracy in work results. |
|
Working Conditions |
|
Individuals may need to sit or stand as needed. May require walking throughout the building as necessary throughout the day and capable of assisting with walks and exercise classes. May include lifting up to 50 pounds for files and other office needs. Primary environment is ambient room temperatures, lighting and traditional office equipment as found in a typical office environment. Available work evenings or weekends as needed. |
